Purpose
The purpose of the questionnaire is to measure children’s academic performance as assessed by their teacher.
Brief description
This is a questionnaire completed by the teacher. After assessing the children in individual subjects such as reading, writing, arithmetic, and spelling, academic performance is also evaluated based on the average or general impression of the child’s performance in these specific academic areas. Finally, the child’s overall attitude/behavior at school is also assessed.
Scoring method
Scoring is based on a four-point scale (1 = well below average, 2 = below average, 3 = average, 4 = above average).
